Client Agreement of Policies & Waiver of Liability

This document outlines the terms, policies, and liability release for massage and bodywork services provided by Nick Wagner Massage & Bodywork LLC (“Practitioner”). Please read carefully. By signing, you acknowledge and agree to the following:

Scope of Practice

1. Practitioner offers massage therapy, myofascial release, and biodynamic craniosacral therapy.

2. These services are not a substitute for medical care and do not include diagnosis, prescription, or medical treatment.

3. Practitioner may refer clients to another licensed healthcare provider when needs fall outside this scope of practice.

Client Responsibilities

1. Clients must disclose health history, current medical conditions, medications, and injuries that may affect treatment.

2. Clients agree to inform the Practitioner of any changes to their health status before each session.

3. Clients experiencing contagious illness must reschedule.

4. Minors under 18 require parental consent. Clients under 16 must have a parent/guardian present; for ages 16–17, parent/guardian presence is optional.

Professional Boundaries

1. Sessions are strictly professional.

2. Any form of sexual harassment, misconduct, or solicitation will result in immediate termination of the session, and the full fee will be charged.

3. Practitioner reserves the right to end a session at any time for safety, health, or professional boundary concerns.

Scheduling, Cancellation, and No-Show Policy

1. 48 hours’ notice is required to cancel or reschedule.

2. Cancellations with less than 48 hours’ notice are charged the full session fee.

3. A client who does not arrive or communicate within 20 minutes of the scheduled start time will be considered a no-show; the full fee applies.

4. Exceptions are granted in cases of illness or genuine emergencies.

Late Arrivals

1. If a client arrives late, the session will end at the scheduled time.

2. If the Practitioner’s schedule allows, the session may be extended, but this is not guaranteed.

3. The full session fee still applies.

Payment Policies

1. Payment is due at the end of each session unless otherwise arranged in advance.

2. Accepted payment methods: cash, check, card, Venmo, Zelle.

3. Practitioner does not accept insurance billing but can provide a superbill or invoice for reimbursement.

Waiver of Liability & Indemnification

1. The client understands that massage therapy and bodywork may produce temporary soreness, tenderness, or other minor side effects.

2. The client affirms that they have disclosed all known medical conditions and will continue to update the Practitioner regarding any changes.

3. The client agrees to indemnify and hold harmless Nick Wagner Massage & Bodywork LLC and its Practitioner from any and all liability, claims, expenses, damages, and losses arising out of participation in massage therapy, except in cases of gross negligence or intentional misconduct.

4. The client understands that bodywork is a cooperative process and agrees to communicate immediately regarding comfort levels, pressure, pain, or other feedback during the session.

Acknowledgment & Consent

By signing below, I confirm that I:

• Have read and understood this agreement;

• Acknowledge that massage and bodywork are not substitutes for medical diagnosis or treatment;

• Have disclosed all relevant health information;

• Understand and agree to the cancellation, no-show, and payment policies;

• Release Nick Wagner Massage & Bodywork LLC from liability as described above.
